What is the event?

Expert Parent Program | Transition from child to adult health care: What you need to know as children reach adulthood

Moving from a Pediatrician to a GP as the primary point of care

Mental Capacity Act & Consent

Best interest decision making & shared decision making

Reasonable Adjustments & Equality Act

Adult teams that you should know about, including: CLDT (Community Learning Disability Team) & HFT (Health Facilitation Team)

Care Plans & Hospital Passports

Other areas discussed will include outcomes, looking after yourself, levers and laws.

You will also be signposted to Continuing Health Care, but this can also be looked at during the workshop, as well as Transforming Care (CTRs, DSR, etc.)
